Pediatric Rehabilitation Professionals
Our exceptional team of pediatric rehabilitation professionals works collaboratively to assist each child in achieving maximum function, productivity and independence.
The nursing team at Weisman Children's Rehabilitation Hospital(WCRH) is trained and specializes in pediatric rehabilitation. We are committed to improving quality of life for children and adolescents with disabilities and their families. Our team includes registered nurses, licensed practical nurses, nursing assistants and patient companions. The registered nurse provides leadership in the coordination of care for their patients, initiates and participates in patient and family teaching and works collaboratively with other disciplines on the rehabilitation team in achieving optimal standards of care.
Our staff of physical, occupational, speech/language and recreational therapists have extensive pediatric experience, as well as specialty training and certifications. We provide state of the art rehabilitative interventions that result in advancing your child along the road to recovery so that they can maximize their potential as they return home to the community.
Your child will have an individualized therapy program tailored to meet their specific needs. They will have the opportunity to master skills in 1:1 therapy sessions as well as in group activities. We will be with you and your child every step of the way.
We encourage family members to participate in therapy as much as possible so that you can help in the recovery process.
The social work department provides ongoing support and assistance to WCRH patients and their
families. The social worker meets with each patient and their caregiver(s) upon admission to WCRH to
obtain information and ascertain how the social worker can best provide assistance to the family.
The social work department is able to assist families with an array of tasks including completing
paperwork, obtaining funding for equipment, applying for Medicaid and locating support groups.
The social worker is available on a full-time basis Monday through Friday.
Admissions and Case Management
The admissions and case management department at WCRH is responsible for helping patients from the point of referral to admission to discharge. Our nurse liaison starts the process going out to the referring hospitals to meet with the patient and their families. Our admissions team evaluates referrals for acute rehabilitation appropriateness. Once a patient is deemed appropriate our admission coordinator verifies benefits and obtains prior authorization. After patients are admitted to WCRH , case management has weekly team meetings to assess the patients progressand update the families and insurance companies. Once a patient is medically cleared for discharge our discharge planner makes referrals and helps coordinate and facilitate a safe transition to their home or next level of care.
